Facility Waiver & Registration
We kindly request that all participants of Best Choice Fieldhouse complete our facility waiver form prior to participating in an activity. Coaches, teams, and athletes can access our facility waiver form and register online for certain programming and events through our AthleteTrax platform below.
RULES AND POLICIES
1. FACILITY USE:
a. Licensee shall always conduct itself so as not to interfere in any way with the operation of the Facility
by Licensor.
b. Licensor shall have full and final authority as to the use of the Facility and reserves the
right to change Licensee’s reservation allocations in Licensor’s sole discretion.
c. Unless otherwise noted, the Facility use license includes surfaces listed in this Agreement. For the
avoidance of doubt, the Permitted Use does not include office areas, training areas or gravel parking
spots on side of the building.
d. Licensee shall not admit a larger number of individuals that can lawfully and/or safely occupy the area
of the Facility utilized by Licensee.
e. Licensee shall maintain the cleanliness of the Facility in the condition provided by Licensor and shall
dispose of any accumulations of trash and debris in the trash receptacles provided by Licensor.
f. Appropriate footwear is required for all participants. No hard or black soled shoes are permitted on
Facility courts.
g. Licensee must enforce Licensor’s policy of NO GUM, including on courts and turf. Players, coaches, and
spectators must be advised of this policy before the Event.
h. If applicable to the Facility and the Permitted Use, Licensee will permit no chairs, movable seats, or
other obstructions to be or remain in the entrances, exits, or passageways of the facility, and will keep all
such areas clear.
i. Licensee shall not be permitted to “sublease” this Agreement or any portion of the Facility without
Licensor’s prior written permission.
j. No portion of any sidewalks, entries, passage, vestibules, halls, or ways of access to public utilities of
the facility shall be obstructed by Licensee or used for any purpose other than for ingress and egress to
and from the Facility.
k. The doors, stairways or openings that reflect or admit light into any place in the Facility, including
hallways, corridors and passageways shall not be obstructed by Licensee without prior written approval
of the Facility Manager.
2. CONDUCT & SAFETY PROTOCOLS:
a. Licensee will respect and follow all instructions and policies provided by the Facility, verbal or written,
with respect to COVID-19 (and any variants) including, but not limited to, wearing masks inside during
practices, games, and scrimmages; conducting temperature checks; and social distancing.
b. Licensor reserves the right to remove from the Facility any boisterous or disruptive persons.
c. Licensee shall be responsible for ensuring no individual or group attending Licensee’s
Event has in their possession, displays, or offers for sale or give away any type of weapon, explosive,
illegal or illicit products, products or services controlled by federal, state, or municipal licenses, or
products or services deemed by the Licensor to be inappropriate.
d. Absolutely no alcoholic beverages, cans, or glass containers of any kind shall be brought into the
Facility by any of the Licensee Parties. This includes the Facility grounds including the parking lot and
grassy areas around the Facility. Licensee further agrees that neither Licensee nor Licensee’s Parties shall
use or condone the use of alcohol, cannabis and/or drugs in the Facility or permit individuals under the
influence of alcohol and/or drugs in the Facility.
e. Following an incident or accident at the Facility, Licensee must file an Incident Report with the Facility
Manager within twenty-four (24) hours of the occurrence.
3. FOOD & BEVERAGES:
a. If applicable to the Facility, all concession stands and concession sales shall be controlled, and the
proceeds retained, by Licensor and/or its concessionaire(s). Concession stands for sales of items other
than food and drink may be operated by Licensee upon the prior written approval of the Facility
Manager.
b. Absolutely no outside food may be brought in and stored/sold inside the Facility or on Facility
grounds.
4. STAFFING:
a. Licensee, at its expense, shall provide any required athletic trainers, medical staff, EMT and/or
ambulance and License shall have the appropriate expertise and training to provide for medical
contingencies or emergencies as may arise during Licensee’s use of the Facility. Licensor will provide at
least one (1) AED at the Facility.
b. To the extent Licensee provides services to minors at the Facility, Licensee represents that criminal
background checks (CORI) on all employees and volunteers have been conducted with no adverse
findings that would disqualify employees and volunteers from working with children, including any
criminal findings involving sex crimes.
c. Licensor is not responsible for ancillary personnel such as referees, trainers, security, etc.
d. A representative of Licensee must be always present during the Program/Event. Licensee shall
properly lock the Facility when it is not open for business. When persons under eighteen (18) years of
age are to be present during Licensee’s use of the Facility, the Licensee shall ensure that sufficient adult
supervision is always present to supervise and protect the minors.
5. PARKING:
Parking is limited to designated onsite parking spaces. Any additional parking, shuttle service, or
personnel necessary to accommodate the Program/Event may be billed to Licensee and must be
coordinated with Facility management at least thirty (30) days prior to the Event.
